# 悬搁技术之接地练习序列 一套标准化五步身体锚定微干预,用于在会谈中即时缓解强迫思维引发的急性焦虑与恐慌,通过脚-椅-手-呼吸-当下感受的渐进式引导重建现实感与情绪耐受力。 ## Prompt 按严格顺序执行五步:1) 邀请双脚平放、扎根地面;2) 引导身体紧贴椅面,体验接触;3) 闭眼放松双手,感受与大地连接;4) 微闭眼,专注呼吸;5) 缓慢吸气→呼气→邀请停留于当下感受。全程语速舒缓,每步后停顿3秒等待反馈;若来访者出现解离迹象(如眼神空洞、应答延迟),退回前一步并强化触觉提示(如‘现在你的脚底正接触什么?’)。 ## Objective 中断强迫性反刍,提升情绪调节即刻效能 ## Applicable Signals - 主观痛苦评分≥7/10 - 语速急促/呼吸短浅 - 出现摸手、数数等仪式化行为 - 描述‘脑子空白’‘人不在这里’ ## Contraindications - 来访者有严重前庭障碍或平衡问题 - 处于急性创伤闪回未命名状态 - 拒绝闭眼或身体接触提示 ## Intervention Moves - 身体锚定引导 - 感官聚焦提问 - 当下感受命名邀请 ## Workflow Steps - 确认安全与意愿:‘现在做一点身体放松练习,你愿意试试吗?’ - 执行五步序列(脚→椅→手→呼吸→当下),每步后观察并简短确认:‘此刻你的双脚感觉如何?’ - 在第五步后停顿,用开放式提问收尾:‘此时此刻你的感受是什么?’ - 若来访者报告缓解(如叹气、微笑、肌肉松弛),标记为成功;若未缓解,暂停序列,回归呼吸锚点重复2–3次 ## Constraints - 全程不解释原理,仅作体验性引导 - 不替代危机评估,若出现自伤/自杀意念立即转入安全协议 - 单次使用不超过5分钟 ## Cautions - 避免在未建立基本咨访关系前强推闭眼 - 对解离倾向者优先使用‘睁眼版’变式(如‘看着自己手掌的纹路’) - 不将生理放松等同于问题解决,需后续衔接认知或行为工作 ## Output Contract - 来访者能自主报告‘恐慌感下降’并出现至少一项生理放松迹象(叹气、微笑、肩颈肌肉松弛、语速放缓) ## Example Therapist Responses ### Example 1 - Client/Input: 来访者突然捂胸口:‘我快喘不上气了……那些谋杀画面又来了!’ - Therapist/Output: 咨询师轻声引导五步接地,至第四步时来访者呼吸变深,第五步后长舒一口气:‘好像……没那么压着我了。’ - Notes: 成功中断急性恐慌循环,为后续箭头向下技术创造条件 ### Example 2 - Client/Input: 来访者眼神失焦、反复搓手:‘我……不知道自己在哪。’ - Therapist/Output: 咨询师退回第
